The Howard County Food as Medicine team is getting ready to launch its next cohort starting Sept. 2, and it is accepting referrals now. Every Tuesday from Sept. 2 through Oct. 28, this free-to-participate diabetes education program is offered to adults living with or at risk for type 2 diabetes who are experiencing food insecurity.

The sessions will run from 11:30 a.m. until 1:30 p.m., and will take place in the Jeff Stout Government Building, 120 E. Mulberry St., Kokomo, in Room 101.

Food as Medicine team members Allison Hillis and Lisa Washington will use the Dining with Diabetes and Diabetes Education Empowerment Program curriculum to help participants better manage their condition. Each session includes a cooking demonstration featuring diabetes-friendly meals, and participants will take home a weekly bag of food, alternating between fresh produce and shelf-stable pantry items aligned with the recipe cards received during class.

Referrals into the program are accepted through Aug. 25. To submit a referral, share this article with individuals who may like to learn more about the program. The QR code included in this article leads to the team’s Interest & Eligibility Form. Or simply click here.

Once this form has been completed, the Food as Medicine team will reach out to the individual to move forward with the registration process.

Alternately, healthcare providers may initiate the referral on their patient’s behalf, by completing the referral form included at the end of this article and faxing it to 765-456-7000.
